The upgrading of your items is an essential element of equipping your character. Upgrades increase the base damage as well as enhancements of items.

Weapon

Once the weapon is at the maximum level of upgrade, it may be reforged with a number of different upgrade types to boost specific stats or to add bonuses and effects. All of these upgrades can be used simultaneously and their effects will depend on how rare the weapon is.
There are two Blacksmiths in the game that can perform these upgrades: Blacksmith Hewg in the Church of Elleh hub area, and Smithing Master Iji in the Road to the Manor Site of Grace. The upgrade materials are both different: Smithing Stones to modify the damage weapons deal and Somber Smithing Stones to alter the standard weapons.
It is generally advised to improve weapon damage first, then armor defense, and finally the other secondary stats that are required by your build. In particular, it's common to see melee druids enhancing their weapon before they upgrade any other gear, because it can increase DPS. This is especially true for enchantments that can enhance a weapon's stats and damage.
Armor

This trinket, in its current level, increases the health gained by drinking healing potion and wells of life by X%, and also gives mind sight to enemies within Y tile. This is not a stacking feature with the Heightened Senses.
Skull Cavern is where you will find Trinkets after you have completed the Mastery Cave. You can find them after defeating Monsters, and in chests and crates. They aren't found in the Mines or Volcano Dungeon.
Place the trinket into the Anvil when it needs to be upgraded. This will affect the trinket that is random that will either enhance or intensify its effects. You can reorge the Trinket as many times as you want, but it will always have a new effect.
You can also upgrade your Trinkets by placing them in a Magical Catalyst located at the Alchemy Station. This will cost you 6 energy, however it will increase the power of the trinket by a tiny amount.
