Costs
There are a few expenses associated with item upgrades. The first is the cost to perform the upgrade. The cost is based on the new ilvl of the item and the number of perks it gains and can be costly. Upgrades to a higher level (such as Explorer to Champion or Veteran to Heroic) can be costly.
The second is the cost of materials needed for an upgrade. These items are typically Power Dices (dropped from monsters 21+), Minerals and Erons, or specific Mists Of Pandaria or Warlords Of Draenor Quests. The amount of these materials required is available in the inventory of an upgrade vendor.
Upgrading an item is complicated and requires time. Based on the magnitude of the upgrade, it can take up to 8 hours. The base stats of the upgraded item are calculated based on its new ilvl as well as other properties. This calculation determines new Armor Class bonuses, damage dice, and other important stats. In some cases the upgrade will provide additional perks that can be selected from a list of available benefits.
In rare instances the upgrade will only replace the highest-costing perks. You can also add additional materials to the upgrade to increase its chances of success. These materials are displayed in a separate window above the + button. There are however limitations to this, and the maximum amount of material that can be added is displayed just above the + button in the user interface.
Another way to increase the odds of an upgrade achieving success is by using an item known as a Blacksmith Blessing or an upgraded version of it.
